San Nicolao is a church within the medieval fortifications of Monsummano and features a single nave with wooden roof structures. The semicircular apse remains hidden from outside view as the sacristy obscures it.

San Nicolao is a Romanesque church datable to the 12th century, recorded in the 1260 catalogue of Lucca churches and built inside the castle of Montesommano. It was formerly a suffragan of the parish church of San Pietro di Montecatini Alto, and its status as a parish church was suppressed in 1786.

The church appears as a place of medieval devotion with its simple interior layout. A wooden crucifix from the 14th to 15th century shows the skill of local religious craftspeople from that era.

The church stands inside the walls of the castle of Montesommano, on the hill of Monsummano Alto above Monsummano Terme. The separate chapel beside it, with a sail bell tower, now houses a restaurant. The bell tower on the right is open at its base through a vaulted passage, so you can walk beneath it.

The bell tower stands on the right side of the building and was originally part of the castle's defense system. This dual purpose shows how religious and military functions were intertwined in medieval fortifications.
