The ward spans 9.75 square kilometers in the northeastern section of Osaka city with residential neighborhoods and commercial districts along train lines.

Higashisumiyoshi-ku emerged as an independent ward on April 1, 1943, when it separated from the original Sumiyoshi district during Osaka's administrative restructuring.

The name Higashisumiyoshi reflects the area's position as the eastern portion of the historical Sumiyoshi district, maintaining connections to local heritage.

Multiple rail stations serve the ward, including Tobu-shijo-mae on JR West Kansai Main Line and several stops on the Kintetsu Minami Osaka Line.

Nagai Park within the ward contains both a botanical garden and stadium, forming a substantial green space in this urban district.
