The original structure was constructed under Emperor Aurelian in the 3rd century.

Porta San Paolo is often associated with the Via Ostiensis, the beginning of the road to the great ancient port of Ostia Antica.

Today, the gate houses the Museum of the Via Ostiensis.

The arch of the gate has featured in various historical events including its defense by Belisarius during the Gothic War.
