The military cemetery in Chlum has a rectangular plan of 838 m², with its entrance on the shorter eastern side, about 250 metres west of the Church of the Transfiguration. Opposite the entrance stands a marble base topped by a bronze sculpture of a sleeping lion, the monument of the Prussian 1st Guards Infantry Division. A cast-iron cross on a sandstone base honours all fallen Austrian and Prussian soldiers, while a broken sandstone column commemorates the Austrian 46th Infantry Regiment.

The cemetery was established after the Battle of Hradec Králové on 3 July 1866, in which nearly 8,000 soldiers died and over 23,000 were wounded. The dead were buried on the battlefield, and monuments were erected almost immediately. In 1888 the Committee for the Maintenance of Monuments on the Hradec Králové Battlefield was founded. The cemetery is part of the protected battlefield area declared in 1996.

Every year in early July, commemorative ceremonies mark the anniversary of the 1866 battle, culminating in a large battle reenactment on the battlefield. At the ossuary, remains found at various spots on the battlefield are deposited each year on the anniversary. The cemetery holds the grave of Prussian Lt. Col. Heinrich Gottfried Ernst von Helldorf, commander of the 1st Guards Rifle Battalion, under a richly decorated neo-Gothic sandstone tumba (monument no. 198). Nearby, a sandstone pyramid commemorates thirty Prussian soldiers and one Austrian lieutenant (monument no. 212).
