Hancock County extends across the northern tip of West Virginia, with the Ohio River forming its western boundary and Pennsylvania bordering the eastern edge.

The county formally separated from Brooke County in 1848 and remained part of Virginia until West Virginia gained statehood during the Civil War in 1863.
