The Sanctuary of Our Lady of Montallegro is a minor basilica and shrine situated on a hilltop near Rapallo. The church features Gothic Revival architecture and houses religious artworks, including a Byzantine painting and baroque decorative elements.

The sanctuary originated in 1557 when a farmer reported an apparition of the Virgin Mary that prompted the construction of a church at this location. The structure was later redesigned in the 19th century with Gothic Revival style and architectural modifications.

Our Lady of Montallegro has been Rapallo's patron since 1739, and since 1948 the town coat of arms has carried her monogram, the intertwined M and A, between two griffins. The hymn Splende in alto, written by the priest Giovanni Battista Campodonico, is sung in her honour. The Byzantine Dormitio Mariae panel left by the apparition is kept in the silver pavilion of 1743.

The sanctuary stands at about 612 m above sea level, on the hill above Rapallo. It can be reached on foot by trail or by the cable car that climbs from the town; the German text's rack railway does not exist here. The climb on foot takes considerably longer than the cable car ride.

The spring that believers say began flowing beside the rock of the apparition is not in a Saint Joseph chapel: the sources place it on Mount Letho, next to the Byzantine Dormitio Mariae image found by the parish priest and his group. The two minutes of 6 August 1558, drawn up by vicar Egidio Falceta for Archbishop Gerolamo Sauli, record cures reported there: the blind seeing, the lame walking, the possessed freed.
