Built in 1762 as a Greek Catholic temple, the church underwent multiple transformations, including its conversion to Roman Catholic use in 1986.
The church interior contained two notable iconostases until the mid-20th century, reflecting the religious traditions of the Lemko community.
Located in the Subcarpathian Voivodeship within Jasło County, the church stands as part of the regional Wooden Architecture Trail.
The building survived the Tylawa Schism of 1927 when local residents converted to Orthodoxy but continued using a chapel in nearby Świątkowa Wielka.
