Originally constructed in the late 13th century under Federico I da Montefeltro, the tower underwent complete reconstruction during the first Florentine dominion in the 15th century.

The tower takes its name from the former Church of Sant'Agnese and forms part of the historical defensive walls system of Pisa.

The structure contains multiple small openings of different sizes framed in stone, with evidence of a former wooden walkway accessible through a still-existing small portal.
