The church presents a rectangular structure with a triangular pediment topped by a clock and incorporates a prominent single tower into its main design.
Construction of the church started in 1713, but an earthquake in 1717 caused significant delays, leading to the first mass being held in 1765.
The church maintains an annual Pachamama procession where the image of Our Lady of Guadalupe moves between the parish church and a nearby mount.
The church stands near the main pilgrimage route leading to the Império of Our Lady of Guadalupe, making it accessible for religious gatherings.
Inside the building resides an organ from 1775 by José Leandros da Cunha, marking it as the second oldest organ in the Azores archipelago.
