The stone church features limestone walls, lancet arched windows, a rose window above the entrance, and an octagonal steeple atop the belfry.
Built in 1880 as a mission church for German settlers, the church replaced an earlier log structure that burned down across the road.
The interior displays original wooden pews, a Gothic altar from Milwaukee, and walls decorated with stenciled designs including stars, lattice, and tulip shapes.
The church now operates as a wedding chapel and public museum, with regular visiting hours during summer Sundays through the Sauk Prairie Historical Society.
The cemetery contains graves dating to 1862, featuring wrought iron crosses and stone markers organized by German, Irish, and English family sections.
